20-30 minutes: $52.50
45-50 minutes: $105.00
70-85 minutes: $157.50
Family Therapy: $110.00
Group Therapy: $60.00 per group

For other e-mail the fee is $50.00 per exchange for my reading and responding to your personal
questions and concerns.
You are responsible for checking your eligibility for insurance benefits. Some policies require pre-authorization for mental health outpatient services. Some companies require that your primary care physician refer you. Other restrictions may apply. I will be happy to assist you where possible.
You will want to know:
1. Name of person you spoke with.
2. Your deductible for the year.
3. Have you met the deductible, if not how much remains.
4. What are the total sessions allowed?
5. Is precertification required?
6. Am I an in-network provider or out of network.
7. If I am in network, what is your co-pay.
8. If I am out of network , what is your percentage.
Cash, check and all major credit cards accepted for payment.
Please give 24 hours notice if you have to cancel or reschedule your appointment. A $50 fee will be charged for failure to give notice.
